RULES FOR THE MOST BEAUTIFUL EYES CONTEST SPONSORED BY WBWB-FM, WHCC-FM AND CLARITI ADVANCED VISION CORRECTION

1. Current employees of WBWB-FM, WHCC-FM, Artistic Media Partners, Inc., Clariti, Inc., their subsidiaries, agents, and representatives and their families and household members are not eligible to participate in the contest.
2. To participate in the contest, no purchase is required.
3. Instructions for submitting your entry and how to vote are available on the contest Website.
4. Participants may enter the contest only once. Anyone found to have multiple entries will be ineligible to win.
5. All contest participants must be at least 21 years of age.
6. Entries must include the contestant's legal name, their current mailing address, email address and daytime telephone number.
7. All entries must be received prior to October 5th, 2009 at 5:00PM.
8. All entries become the property of WBWB-FM, WHCC-FM, Artistic Media Partners, Inc. and Clariti, Inc. and will not be returned or acknowledged.
9. Participation in the contest constitutes permission of all contestants and winners for WBWB-FM, WHCC-FM, Artistic Media Partners, Inc. and Clariti, Inc. to publicize and/or broadcast the contestant’s name, image, photograph, character, likeness, voice, entry and the fact that he/she participated in and/or was a winner in the contest. All participates waive any right to compensation.
10. WBWB-FM, WHCC-FM, Artistic Media Partners, Inc., or Clariti, Inc. are not responsible for lost, delayed, misdirected, incomplete, illegible entries or entries not meeting the standards for picture submission, Internet or equipment failures, any and all technical problems, human errors or acts of God.
11. WBWB-FM, WHCC-FM, Artistic Media Partners, Inc. or Clariti, Inc. may designate, at their sole discretion, judges for the contest. The judges will determine and validate the contest winners. By participating, contestants agree to be bound by all contest rules and the decisions of the judges.
12. Prizes awarded towards the cost of iLasik are valid for procedures performed by December 31, 2009.
13. For contest winners determined by a random drawing, odds of winning are determined by the eligible number of entries.
14. The entry that has received the most votes by 5 p.m. on October 5th, 2009 will be the "Grand Prize Winner" and will receive a certificate that can be used to receive, at no cost, the iLasik procedure performed on both of the individual's eyes.
15. The first runner-up will receive half off their iLASIK procedure. The second runner-up will receive $2,000 off the price of their iLASIK procedure.
16. Other prizes may be awarded during the contest period and details regarding winning these prizes will be announced on-air.
17. Prizes may be picked up at the medical offices of Clariti, Inc. during regular business hours. To receive a prize, all winners agree to have a complementary, no-obligation, LASIK evaluation to determine the individual's suitability for having i-Lasik.
18. Identification, i.e. a valid driver's license, is required to participate in the contest. All contest winners will be required to validate their identity and may be required to execute an affidavit of eligibility release/prize acceptance form and a W-9 tax form. Noncompliance may result in a winner's disqualification or an alternate winner being selected.
19. Suitability of receiving the iLasik procedure will be determined by Dr. Matthew Fornefeld, in his sole discretion. In the event that a winner is not a suitable candidate for i-Lasik, the value of the prize may be applied toward the cost of another advanced vision correction procedure currently performed at Clariti Advanced Vision Correction, such determination of suitability to be made by Dr. Fornefeld, in his sole discretion.
20. Prizes must be accepted as awarded, and cannot be transferred or assigned. No cash awards in lieu of prizes are allowed. Participants waive any and all claims of liability and agree to indemnify and hold forever harmless WBWB-FM, WHCC-FM, Artistic Media Partners, Inc. and/or Clariti, Inc., their affiliates, employees, agents, vendors, suppliers, their families and members of their households from any accident, loss, and/or injury occurring as a result or consequence of contestants' participation in the contest and/or all winners acceptance or use of prizes awarded as part of the contest.
21. WBWB-FM, WHCC-FM, Artistic Media Partners, Inc., and Clariti, Inc. do not warrant any of the products, materials or services that a winner receives. The materials, services or products that a winner receives are provided "as is".
22. The contest is subject to all applicable federal, state and local laws and is void where prohibited or restricted by law.
23. These rules may be changed or altered at any time without prior notice.
24. WBWB-FM, WHCC-FM, Artistic Media Partners, Inc. or Clariti, Inc. will issue a 1099 tax form for all prizes valued at $600.00 or more, including multiple cash prizes to the same winner adding up to $600.00 or more over the course of the calendar year. Winners are solely responsible for all applicable federal, state and local taxes. By claiming prize(s), winners agree to accept the estimate of fair market value placed on prize(s) by WBWB-FM, WHCC-FM, Artistic Media Partners, Inc., and/or Clariti, Inc.
